* Housekeeping must be less than 20% of the caregiver's working time during each shift

Includes all services in Level I plus:

• Personal hygiene assistance
  Bathing, grooming, shaving, hair, oral hygiene, skin care, dressing,
  Help with elastic stockings and other supports
• Medication reminders
• Toileting
• Incontinence care
• Minimal transferring

Includes all services in Level I & II plus:

• Moderate to maximum transferring
  From bed to wheelchair, wheelchair to car, etc.
• Assistance with other specialized equipment such as Hoyer lifts
• Provide Hospice assistance and care to those with Parkinson’s disease and moderate to advanced levels of dementia, including Alzheimer’s disease.
